Case Management and Supports Coordination
Managing care is a collaborative process of assessment, planning, facilitation (linking and brokering), care coordination, evaluation, and advocacy for options and services. We are best able to meet an individual’s and family’s comprehensive health needs through communication and available resources. This is done to promote quality, cost-effective outcomes.
Our care management team can assist in care transitions, medication oversight, and even coordinate technology for those who need it. For example, for someone who has undergone a leg amputation, we can coordinate the installation of shower or toilet grab bars and/or house entry ramps.
